The answer in the games is, large amounts of radiation turn some people into ghouls, and it's something to do with mutations causing regeneration, but there's no real way of knowing who will turn into a ghoul and who will just die. They go feral because of their brain decaying, and that's more likely if they're isolated, but being social is not guaranteed to stop it.

Feral is kinda just a really old burned dude with advanced alzheimers, basically. Feel good about shooting them as canon fodder now?

I don't play the games, but I assume that they kill the player character if you don't kill them first. Honestly it might be something of a mercy if that's how it is. In the show, it shows ghouls in the process of going feral and it comes across as really unpleasant for them.

Bingo. Feral ghouls are hostile to everything except other ghouls, and you don't get to be a ghoul.

It's never really been established why some ghouls go feral while others have been around since the Great War and are still mentally undamaged.

My theory is that ghouls can survive indefinitely - radiation heals them - but the healing's never perfect. Hence the decayed appearance. So if they suffer brain damage or have some other neurological issue, they can heal from it, but the damage is still done and they become 'feral'.
