It's a lot easier when you have the author telling you what one or both of them are thinking and feeling at the time. But for movies/tv, you need good actors, writers, directors, etc. I watched a tv show with a dubcon scene and a completely consensual scene between two different couples. The consensual one was the one that felt like a rape scene because the acting was off. Not exactly an issue with a book or a fic.

I watched a pretty enjoyable movie. But than they had this dubcon scene, and it was so obvious that neither director neither actors knew what they were doing. Instead of two grieving people we had got a rape scene. This one small scene forever soiled me the movie. I was so annoyed I vented to my friend for like 15 minutes.
