I read the Doctor's passionate response as him being pained that he couldn't save people, which doesn't happen as often as he would like. Especially since all he had to do to save those people was give them a ride. Instead, they chose a slow, agonizing death because he was different.

It reminded me of "Night of the Doctor," when a woman refused to board the TARDIS and let herself die because she hated the Time Lords and how their war with the Daleks was destroying the universe. The Doctor was so upset by Molly's, I think her name was, choice to crash with the ship they were on that he stayed and died with her. The ending of "Dot and Bubble" had a very similar outcome, reaffirming how much it hurts the Doctor not being able to save people. And the mindless reason he couldn't save those people was just salt on a very old wound.
