The only person I've ever met who was open about being into that, it didn't make me think she was a bad person who would hurt anyone for real, but it did make me think she was misanthropic and not want to talk to her much. We were in the same fandom circle and not only was she into gore and torture, she also made a point of hating characters who were nice/good people and not fucked-up. She herself was pretty friendly, but her tastes convinced me it was an act and she was secretly looking down on the rest of us for not having as fucked up taste as her.

I've come across people who were similar to the person you spoke of.
Generally, they were nice, but they seemed to go out of their way to let people know all the dark stuff they were into, and they would be very contemptuous when speaking of "happy, chirpy smiling people who act like they're sOoOoOo happy with life".
It felt like they were looking for someone to be concerned about the stuff they liked and said. As if they wanted to offend people. I can only assume so that they can feel better than other people.
