TLDR
Basically in my view sometimes authors CAN do this.
Granted 90% of the time when fans complain about this... it's pretty questionable, and usually they're bwaaing about their ship being sunk or whatever and it's NOT technically OOC in reality. It's actually more so that either the development or some other factor was DISAPPOINTING perhaps to the reader. Kind of like how a canon character you hate isn't -necessarily- a Mary Sue no matter how many times people use the phrase- they say Mary Sue, but what they really mean is they dislike the character immensly or the character overall leaves a bad taste in the mouth. I think a lot of people misuse that sort of terminology sometimes. Just because you dislike where the character went in terms of development (or alternatively how they didn't develop in some odd cases) doesn't mean it's NECESSARILY OOC.
But an author, the actual creator CAN do this- kind of like in my own example.
Stories are generally both character and plot driven as well as set around the world they're in. Each three hopefully affect each other in some way and tell us something about each of them in some way. Sometimes it's hard to get that balance though. A talented person can mesh the concepts together and make them flow naturally but even there mistakes can happen and as stated by another anon above, sometimes readers think more deeply on copncepts the author didn't even KNOW they inferred. That's okay usually, and can make things a lot more interesting. An odd plot hole here and there occasionally happens anyway, and no story is ever perfect, but hopefully they mostly try their best.
Sometimes however an author maybe really desires to use a scene and a concept. And I mean REALLY desires it, it seems amazing to them maybe, perhaps to them it holds the whole or a major part of the plot/subplot they're going for, or they simply think it would be really interesting and exceedingly necessary to the plot or the message they're trying to send. For instance in my own personal example, I really wanted one of the protaganists to become a traitor- and for a time it was that male character. Thankfully I realised a female character fit the bill better than he did. So I got to keep the concept- though it did change quite a few things too.
But what if no-one had fit that bill? What if instead of either going back and changing the character a little bit (at least) from what he became, I kept the character as he was and forced him into that role? It would have been tredding the line of OOC in the end and would not have made sense.
And the thing is in the publishing perhaps of a series for example, this could be a very real possibility, especially if you publish as you go along instead of completing it first and maybe don't re-read or whatever- things can get forgotten or mis-remembered. (This is where plot holes can also come into play too). Sometimes, but not all the time. Because you can't change the character all THAT drastically without a due explanation in that senario- well you can of course, since it's yours, but readers would certainly be in the right to question it. In fact if I was honest in that situation, it just DOESN'T fit this male character I have created. The only other person even less likely is another female in the group probably. And once again- my characters. And if I'd kept everything the same and not changed a thing- OOCness. Oh yes.
Of course I could shoehorn the guy into the role anyway like I said, if I really wanted to. But that would be pretty bad in my view.
OOCness CAN occure in canon characters just likie the idea plot holes can happen. It's just not as common perhaps as fandom would have you believe (unlike plot holes), plus OOCness is FAR harder to prove and is a lot more subjective than plot holes. Just like the concept of a canon Sue perhaps, people can misuse the term a lot without realising it.
Not liking something in relation to a character does not naturally equal to OOCness or canon Sues.
