Originally, yaoi was used for Japanese fandoms, slash was used for Western fandoms. That's how I still use it. However, a lot of people involved in anime fandom also use yaoi when referring to Western fandoms. Or they come up with convoluted, complicated definitions that nobody needs. But talking about yaoi in a Western fandom (even if it's animated) makes you look like a crazy anime fan. (Don't know what you look like when you talk about slash in Japanese fandoms, because I've never seen that happen. ;))

It depends on the fandom. It can have neutral connotations, but from my experience, it usually seems to create an impression that the person who uses the term is distancing themselves from the yaoi genre and the problematic stuff that characterizes it.
