If you have a good enough budget, and Gaga does, you can photoshop frame by frame. There's also general distortion effects that let you move bits of body around, and all mainstream videos have at least some tweaking to make the stars appear thinner. Watch Gaga's Bad Romance video, where the technique is used to make her look creepy as hell:





In the first screen she's been slimmed down to the point where she doesn't even look human; in the second her leg has been digitally repositioned and lengthened, and in the last she has huge anime eyes. No-one could physically have a body shaped the way her body is here. If you honestly think this is what Gaga really looks like just because you don't happen to know how video editing works, then I'm sorry.
