Just a warning, some spoilery stuff below (for HP, BtVS, TVD, and Heroes):
The usual suspects in HP: Umbridge for Harry's hand carving, Draco for being a brat (and calling Hermoine a Mudblood), and I was pissed at the Kreacher stuff in Book 7 because he had lied about Sirius being at the Ministry. Now I'm not as upset about it, and I understand why he did it, but it still seemed too... IDK, easy and kind of trite that he completely flipped sides in such little time. I also think the moment Snape called Lily a Mudblood was when he lost her, and rightly so.
From The Vampire Diaries: Damon. Just Damon. I could forgive the early crap with Tyler, Bonnie, and Jeremy because nearly everything in the first few episodes seemed really shaky and experimental, but rather than grow in a positive way like the others mostly have, Damon's constant shuffling between the poor heartbroken romantic hero and the selfish uncaring villain, hasn't endeared me at all to him; it's just made me hate him more. The second he snapped Jeremy's neck, I could never see him and Elena involved romantically, and I think both their characterizations have suffered to make headway for the Delena ship since then. It's not just that instance, it's that he basically has gotten away with: abusing/raping Caroline, abusing/raping Andie, trying to kiss Elena and feeding her his blood without her consent, intimidating Bonnie and seriously trying to kill her, etc. He could end up doing the most badass, selfless thing in the world, like sacrificing himself for Mystic Falls, but unlike my reaction to Spike's death, I'd still be cheering for Damon's rather than feeling sorry for him or finally seeing him as a genuine hero.
Heroes: Sylar. It's mostly due to him in Volume 3 that made me wish he'd just died in the S1 finale. I could live with the careful balancing act of him being a tortured soul, fun/interesting character to watch, and a ruthless killer in the first two seasons, but when the writers tried pretending none of it was his fault and gave him an actual romantic storyline, I lost the love I had for the character, and I never stopped side-eyeing the creator/writers from then on. Sylar's 'heroics' in the last couple episodes of the series made me roll my eyes.
I have a couple more, but this is already long, so I'll leave it at that. Sorry if the characters are from shows you didn't watch! And I'd be interested if you want to discuss any other characters from HP (or wherever).
ETA: I deleted that BtVS paragraph because I realized those characters don't fit what we were talking about.

I never liked Tristan from Stardust because I was far more interested in Dunstan, his father, who never treated women as a PRESENT to give to another woman, or tied them up. But the moment that solidified my dislike for him was when he left Yvaine to go talk to Victoria without thinking about leaving her a note, and instead telling the creepy innskeeper a message that got distorted. Petty, maybe, but significant.
I had a moment while reading Goliath of the Leviathan series where I suddenly despised one of the main characters so much I stopped and made a fansecret about it. (I think this was several weeks ago now)
I actually left the Naruto fandom years ago over my dislike of Sasuke, and this was before he went evil.
Jeff from Rear Window, while I find myself rooting for his side, acts so much like a step-relative I hate during his argument with Lisa (childishly telling her to shut up and generally being dismissive of her) that it's not until the killer is outside his door that I start feeling any amount of sympathy for him.
In the Illusionist, I was suddenly very angry at Alice for assuming that money would be coming out of nowhere, just to please her want for some pretty shoes. I felt no sympathy for her and her oblivious nature to others suffering and pain and difficulties by the end of the film.
There are others but I'm blanking badly on them right now. I'll add more if I think of them. :)

People not telling each other important things that could prevent so much headaches annoy me, in general. Which is why I can't stand most comedic movies and shows, I guess, what with miscommunication often being central to the plot. :P
Oh, more recently, I watched the Sorcerer's Apprentice, but gave up about half-way through the movie. I felt so, so sorry for that wizard guy for having to put up with the main character -I was so annoyed I can't remember names! I think wizard-guy was named Balthazar-, who was all "oh, hey, sweet, magic, sure sure, I'll make the oath to be learn from you, blah blah blah", and then when Balthazar warns him that the Evil Guy will be trying to kill him and he should take his training more seriously, and asked him if chasing the Female Love Interest of the Hero was worth it, the kid says "sure" and goes after her, and then, when the Evil Guy predictably almost kills him, the kid freaks out! "He tried to kill me! I want out! Never mind that I'm not taking your help seriously and often standing you up just to try to get into Female Love Interest's pants, and that you are being surprisingly understanding and patient with me! I'll just do what I want like a bratty teenager!" And then I gave up on finishing the movie and decided to go do something else.
