OP, there are PLENTY of manga like this if you only look hard enough! But the one author that jumps to mind is Yoneda Kou. Specifically Don't Stay Gold, Tadayoedo Shizumazu Saredo Naki mo Sezu, and Saezuru Tori wa Habatakanai. They're a trilogy (which is still ongoing) and they're some of the best BL manga I've ever read.

Basically these kids meet in high school and get pretty close because one of them is a masochist who has sex with a lot of random thugs and the other one has a scar fetish. So they have a lot of sexual tension, but nothing ends up happening, even though the masochist (who is also quite the sadist) ends up having feelings for the scar-fetish guy. So then they grow up and the masochist guy is a yakuza boss and... wah...

OH MY GOD IT'S SO GOOD PROMISE ME YOU'LL READ IT. Yoneda Kou is just excellent in general and pretty much everything I've read by her has surprised me in a good way ehehehe.

Seconding the Yoneda Kou rec. She's an excellent writer (in terms of storyline/pacing) and her art is fab.
