So, for anyone scrolling past, spoilers below, though I think some of it is more my interpretation of what happened than an objective report. In any case, I'm too lazy for that white text shit, so if you're spoiled that's your problem.

In that scene, Brandon (Fassbender's character) has really started losing himself. His life is spiraling out of control because his sister's renewed presence in his life has started to make it harder and harder to hide his addiction (they live together in a nice but somewhat small apartment together, and he can't really escape her as she doesn't seem to believe in personal boundaries). When they're both home, it's not exactly easy to get off the way he had been before (prostitutes and porn for the most part as far as I could tell), and the shame of his addiction makes him want to hide it from her, so he either can't be home or he has to suppress his addiction when he's there. It's kind of a lose-lose situation for him because both he and his sister know that they both have issues with sex (his addiction and she seems to think that having sex with someone equals a relationship and/or love).
He's also been having trouble at work - his computer was taken in to be cleared of the huge about of viruses in there from the porn he downloaded and watched/looked at in the office (his boss, who also seems to be his best/only friend, seems to think it was a prank or something pulled on Brandon by an intern as far as I could tell, but he still knows about the porn which is bad enough in itself), and Brandon couldn't get it up for the woman he met and dated a bit at the office while he could for anonymous sex with a prostitute. He can't have sex with any sort of feeling behind it anymore. At the point of the threesome, he's seeking any sort of sex he can because his life is so meaningless and such that he looks for any kind of release he can get. It takes place directly after he follows a man into a gay club and gets a blow job from him (Brandon had been shown to be nothing but completely an utterly straight up until then, so it comes across more as an act of desperation than attraction), but it wasn't enough, hence the threesome.
The voiceover is part of the English version, too. It's his sister's voice; she's leaving him a voice message. It's a call back to some of the first few minutes of the movie where she leaves him message after message on his answering machine before moving in (she was calling about moving in with him after a bad relationship fell apart, but he never picks up, so she randomly shows up in his apartment as she has an extra key). The message in the threesome scene is a cry for help from her, and he is avoiding her yet again because he doesn't want to deal with any sort of emotion whatsoever.
Like in the beginning, he never picks up. When he returns home from the threesome after finding the message on his cell phone and listening to it (and recognizing it for what it was), he finds her bleeding out from self-inflicted wounds in the bathroom because her life was kind of out of control at that point, too.
