It was originally Ditzy Doo, it was officially changed to Derpy as a nod to fandom.

No, her name was never Ditzy Doo in canon. There was a Ditzy Doo mentioned in canon, but who never appeared, and some of MLP fandom decided that was the pony now known as Derpy's name. But the closest that ever came to being canon was when someone asked Lauren Faust if they were the same pony, and she said she didn't know, but if Ditzy Doo ever appeared in an episode, maybe they'd make her the same pony.

Was it? Ditzy Doo was an offscreen pony mentioned by name only. Did the FiM team ever confirm that was originally the walleyed one's name?

No; the walleyed one we call Derpy has been jokingly called Derpy since the first episode (she was slipped into the background with the expression as a joke by one of the animators) and Lauren Faust said on her dA that they might call her Ditzy Doo if they gave her some sort of little role in the show. Then in the episode that caused all this waah-waahing, she was called 'Derpy' because that's the most prominent fandom name for her.

So we don't know who the first Ditzy Doo was (it was just a one line joke that she flew north to get the southern birds) because we never saw the pony in question, so fandom joked that she's Derpy (before that name got made canon).

No, Ditzy was a maybe. The referenced a Unicorn named Dizty whom Rainbow had little respect for, but it was never intended to be Derpy.

Once the meme took off, Lauren wondered on her Deviantart if she should fuse them, put a face to Ditzy, but decided that Derpy was already it's own thing and she shouldn't disrupt it. This solidified a chunk of the fandom fanon that says that's her real name now though, but there was never anything official.
