I do think it's possible to revise and rework an AU to the point that it truly is a new story, but the problem is that a lot of authors don't want to put the work into that - It's a lot easier to just find/replace character names rather than ask themselves where they've most strongly borrowed from the source and put a twist on it that's totally new, like make the characters do things that would be totally OOC for the inspiring characters and by extension create different conflicts and so on. If somebody doesn't feel like doing that or isn't inclined to do it because they're too attached to how it is when it's directly inspired by something else, it's probably not a good thing that they're trying to publish it as their own.
