In addition to the new/ascended-bit-character mods, there are a couple mods out there that nuke the gender restrictions on all the romances, and patch the dialogue so it matches your Bhaalspawn's gender. (Pretty sure there's also one that allows you to bypass the check that shuts down all other romances once one's been locked in, but I might be hallucinating that.)

And really, a lot of the replayability of the BG series comes from the modding community in the first place. After a few run-throughs, you've seen pretty much everything the vanilla game has to offer.

Warning about the multiple romance mod, if you use the one that comes with the fixpack, though: it also makes it impossible to shut down a romance on your own. Which can get so creepy (Ugh, ugh, ugh, Anomen apparently trying to manipulate the Bhaalspawn into pretending she loves him after she turned him down made me horrified before I realized that it was the game thinking the romance is still active... not that "reaffirm your love for me or I'll kill this guy!" isn't still pretty freaking manipulative)
