They don't explicitly say in the TV show but in the books there was a distinction between the types of ways the Dothraki have sex.

A Khal would have sex with his Khaleesi only through anal means, it was the Khaleesi's decision to let the Khal have sex in the traditional manner. Which gives Daenarys' pregnancy great significance.

Then..how are the little baby Khals made? I took the stuff in the books to mean they typically had sex with the women face down, on her knees, but not necessarily anal. Just...not facing each other.
