Mine aren't particularly painful (I get some serious pains in my lower back on the afternoon of the second day like clockwork, but other than that it's fine), but I bleed a *lot*. Tampons were completely insufficient, and besides that were also painful to wear. Before I found out about menstrual cups, leakage and staining *was* one of my main concerns. I avoided going anywhere for longer than an hour if I wouldn't have access to a bathroom. I woke up on average 4 times a night to change pads and at the end of the week I'd be a sleep-deprived wreck. And depending on where you are, menstrual cups may not exactly be common knowledge.
All in all, the point is that in a world populated by so many spandex clad ass-kickingly active women there should be a bit of variation rather than 100% 'insignificant enough to never mention or think about'.
