My top favorites (Tom/Harry) are:
Words Fail by Nea Marika
During the fight at the Department of Mysteries on OotP, Harry travels to Tom Riddle's time on accident, but in a twist, everyone in the past knows he is from the future, because the time travel took place in the Ministry of Magic and the Unspeakables got to him first.
This was before Sirius died, and Harry doesn't know about the Horcruxes, or anything that happens after that point in time in the books.
The characterization of, well, everyone, is incredible. The updates are super slow because life got in the way of the author, but the fic is not abandoned. I recommend it anyway.

Soulless Solace
It's a divergence that takes place during Half-Blood Prince, where Harry sees the memories of the Gaunt family and feels sorry for Merope and Voldemort. And things just get a little more sinister from there.

Their Verdict of Vagaries
The end of Deathly Hallows goes a little different, and Harry goes back in time to Tom Riddle's era. His real identity is a secret here, unlike Words Fail, though.
What I like about this fic, and what tortures me, is that for all that Harry is trying to be a good person, every single even in canon's backstory has so far come to pass.
But it's very well written, and it still makes you wonder what will happen next.
It can be very tl;dr, though, and I admit I have skimmed some parts.

Kind of a guilty pleasure, there is underXmyXumbrella
Their stories are... well, fairly self-indulgent (of the hawt yaois variety; that's why I prefer their not-porny fics), and very heavy on the sex, but some of them are interesting. Don't read them if you're looking for something deep, but sometimes they get into some interesting angles of the dynamic that Tom and Harry could have, so they're interesting for that alone. Sometimes Tom and Harry are only characters that have the names and share some personality traits but that otherwise resemble cliché porn stars, but some other times, they really are Tom and Harry. It's very hit and miss and YMMV. vOv
(I do not recommend, for example, All About Your Heart, because of the "sexual harassment is love" vibes in it, but outside of that one fic, the author does acknowledge that that sort of thing is bad. So, um, proceed with caution. I think the author puts warnings in all their stories, thankfully.)

This one takes a leaf out of the Inception book, but it's Harry Potter through and through.

If you're interested in Harry/Voldemort, rather than Harry/Tom, I recommend:
A Snake Named Voldemort.
It's an AU during Half-Blood Prince where there is a change of plans, and Voldemort finds himself at the mercy of Harry Potter.
It is complete.
Freefall:
AU during Deathly Hallows. Harry is even worse at blocking out Voldemort from his mind, and things kind of take a turn for the weird when he finds himself feeling attracted to Voldemort.
It is better than it sounds, really. It has explicit content, so, um, beware of that?
It's notable because Voldemort never stops beings, well, Voldemort.

This one is a one-shot, and not very long, but I like it so much. It ignores aspects of Deathly Hallows and has a different outcome of the final battle, but it otherwise sticks to canon.
The premise is that Voldemort won, but is bored.
Aya Macchiato / Athy / Athey likes to write dark/gray!Harry stories and it shows, but has the self-admitted terrible habit of not finishing most of her stories (and not self-admittedly, she does too much telling and not enough showing, but makes up for it with decent dialogue and the research she puts into folklore and other subjects for her stories).
Truth be told, her stories are more of a guilty pleasure; she has fun ideas, but has this tendency to repeat many plot points and ideas over and over in her stories, and sometimes they're too self-indulgent, but they're fun to read if you're not taking them too seriously.

Twice into the Abyss
There is this one with a very unused twist in a very used premise: Voldemort returns back to his younger self to the timeline of Philosopher's Stone instead of Harry
It claims to be eventual HP/LV, but there have been zero hints of that as of yet (thankfully, considering the ages involved).
There is an annoying amount of typos, but the writing is generally good.

The Portrait
I think this was somewhere undecided between Tom/Harry and Voldemort/Harry or maybe both, and the author decided to rewrite it so the updates stopped, but I recommend

Memento Mori, Memento Amor
Despite the pretentiously clichéd title, the story is good.
When Harry realizes he is the Master of Death and that Voldemort is condemned to Limbo because of his Horcruxes, he's all like screw that, I'm saving Voldemort's soul too.
It hasn't updated in a while, so I don't know if it's still a work in progress or if it's abandoned.

I also liked The Train To Nowhere by Pain Au Chocolat, but I got tired of it after the last few updates. This author's stories annoy me, to tell you the truth. I don't really recommend them. They have some skeevy things regarding sex and abuse, (like, in one AU story, Tom is basically sexually harassing Harry and Dumbledore approved/turned a blind eye to it because "their love would redeem Tom". I clicked that close button so hard, you have no idea), and the author has this tendency to delete stories or stop updating them because of drama/personal reasons, or make them only accessible through pdfs, and it's just not worth it.
...Sorry, I just really wanted to rant about that. I keep seeing their stories recommended and I always held my tongue, so this was a chance to get it all off my chest.

If you're looking for stories of the lemon-y persuasion, there are some decent ones on adultfanfiction.net (hidden in the middle of, well, your typical aff dot net stories), but they tend to be of the dub-con or non-con variety, so I'd rather not go and search for them again, sorry. :(
