And yeah, I totally understand. I like seeing characters suffer for completely non-sexual reasons, and non-sadistic reasons. I usually like it when it happens to my favorite characters too. And that physically almost-but-not-quite pain sensation? I'm pretty sure I know exactly what you mean. There's a little twist in my lower chest, in that little corner where my rib cage draws up to my sternum, like a fist with long nails is digging in and squeezing and twisting. Also, kinda goosebump-y feelings along my neck and shoulders and up into my skull. It may be a bit different for you, but yeah, like almost being in pain, but not exactly. Kind of like an echo or phantom of pain.
I know that sounds TMI, but it's the feeling I get when, like, Faith shot Angel with that poisoned arrow, or when Han was tortured in Empire Strikes Back, or Luke was electro-tortured in Return of the Jedi, or when Ron's leg was broken in Prisoner of Azkaban, or when Harry's arm was stabbed in Chamber of Secrets, or heck, even little things like that bit where Steve was blown out the Grand Central window and fell onto that car in the Avengers. Stuff like that.

You're headed for a steep fall, and it's that second before you hit the ground. You've been hit really really hard, and it's that second where you flinch right before you're struck. It's a flutter-kick of fear, and the warm spread and tingle of heat before the pain blossoms.

It's those long, unsure moments after you've fallen unconscious, and are swimming in that half-formed world between reality and hallucination. It's the feeling of the bones and joints of your body poking through your skin, that long sigh of sickness.
I've gotten hurt a lot, I've learned to like it :/

And because it's just so intense, it's like an excuse for every batshit insane protective and cathartic and vindicating "see how brave/how sacrificing/how cool/how tough and badass/what a poor baby s/he is?" instinct to kick into high gear, because, you know, pain! Injury! You have a reason to feel that way that you can pinpoint! *shrugs* I dunno, I've spent a lot of time analyzing which actions of which fictional characters provide me with maximum emotional torque and why.

Also, that's exactly the feeling I get when I'm on the edge of my seat worried what a character I like is going through.
