They deliberately (and quite obviously) recreated that cover imagery during the closing ceremonies of the Olympics (in the UK, no less), and most people STILL didn't get it. lol This really shouldn't surprise you.

Pretty much, yeah. Not everyone gets the reference or the cultural significance of that cover.

People were probably focusing more on the fact that a guy was tight-rope walking (which isn't part of the cover) than anything else and if they weren't already familiar with the cover image, they wouldn't know why the flaming mannequin was significant.
