I feel you, anon.

My best friend who I've known since we were little kids is more or less a sister to me, but we always get mistaken for lesbians.

Is being mistaken for lesbians worse than if you'd been accused of dating a male friend you'd known for a long time and felt was a brother to you?

I don't think it's better or worse. They both sound like sucky situations.

That being said, I never minded it personally. It was mildly annoying, but other than that it didn't bother me much.

Depending on where you live, it's a thousand times worse. In America, anyway, being gay is grounds for getting fired, beaten, murdered, or having outrageous lies spread around by "family" groups about your "kind" (gay = pedo). High school can be hell for LGBT youth, and many commit suicide.

If people think you like / are dating the opposite gender, usually the worse you'll suffer is some teasing. Same gender = ostracism at best, torture at worst.
