Maybe because they didn't anticipate a friend having a situation where they really needed OP's help or concern? I somewhat agree about the honesty part, though - saying "I have a friend who really needs me right now" is a legitimate reason to cancel plans and real friends would understand. But "busy" is also kind of true since it's something OP feels they have to take care of. It's really more a matter of how much information you want to share with people.

I'm guessing OP makes plans because they want to spend time with irl friends too, and it doesn't sound like they cancel every plan they make.
