It's very cliche, but nails on a chalk board. Even thinking about it makes me cringe. I have to fast forward or turn off movies with that in them (like Jaws). It makes my hair stand on end and make my muscles go so tense I ache.

And, of course, leafblowers. Whoever designed the damn things is a sadistic bastard (okay, not really, but I swear there's got to be a quieter way to do that).
