Honestly, the people I see using the phrase 'casual gamer' as an insult are mostly assholes who play games all day long and write long-winded, poorly spelled forum posts about 'game balance' and 'playing to win,' while being fantastically awful at whatever game/genre of choice they happen to play.
Basically they're failures and the only way they can feel better about themselves is to pretend they're part of an inherently 'better' subculture, and they slap the name 'casual gamer' on players they don't like--the implication being that a 'casual' doesn't understand/appreciate the game the way the ~hardcore~ do.
