That and the fact that in a lot of fantasy TV and movies, the settings/buildings/costumes are obviously influenced (via the influence on the genre of Tolkien, who in turn was influenced by European myth) by our collective idea of Medieval or Dark Ages Europe. So we think we should be hearing European (by which we often mean British) accents to match up with that, even though it's not meant to be a depiction of real!Europe.

I'm from the UK and it used to irritate me too (also costume dramas), but I've started either not to notice or actually to like the use of American accents. If a fantasy setting is based on Classical antiquity, for instance, then it makes sense to have a mix of accents from all over the place, because that reflects the diversity of the settings the fantasy world was influenced by. Rome thousands of years ago would've been like the big cities of today, with populations of different ethnic groups - and of course, its army was drawn from all over the empire. I think it was probably watching the original Conan the Barbarian movie that got me thinking like this, and since then that kind of variety hasn't bothered me...
