I'm American, and I use it interchangeably with "um", particularly when typing, but IRL too.

Most Americans, at least in my region [The South] use it to show uncertainty, or disagreement. For example 'Er, I don't know...' or 'Er, actually I think they might be right...' Where as 'Eh' is used to show apathy. ['Eh, it was alright, I guess.']

omg

I use "er" all the time and I'm American...but I just realized that it's possible that I learned the word "er" entirely from reading the Harry Potter series when I was around 9-10 years old.
