When it comes to fic, I think that very few people actually do. Others, like myself, start with a general idea of beginning, middle, and end.

The longest fic I've written is 28 chapters. When I started it, I had a sense of what I wanted to do, but it wasn't fully fleshed out. Over the course of writing it, however, the pieces started to come together for me; chapters 15 through 28 had actual outlines.

I've found, over time, that that's how I work best when it comes to writing. When I try to plan everything out in advance, I get bogged down in the details, and the actual writing never happens. I have to start writing first, and then lay outlines when it becomes necessary.
