Pretty much this. Unless the scar has a deep meaningful history (usually told in detailed flashbacks) the makeup department won't bother. Heck they forgot to add Castiel's burn mark on Dean's bicep and had to come up with some bullcrap story to explain it and that was a scar they did make a big deal about.

I thought Supernatural was a relatively interesting example. While they don't typically preserve scars (although, sometimes injuries for a while), they have included lines that suggest Sam and Dean do have the expected scars, we just don't get to see them. For example, when Dean was raised from Hell, he mentions that all his scars (and improperly mended bones) are gone now.

So it becomes a bit like suspended disbelief during a play: we know the characters have scars, even if the actors do not. So you can mentally fill in the gap. An odd approach (and clearly because of the logistics of makeup, just like when Shakespeare has characters describe things that cannot be shown on stage), but kind of interesting in that the script acknowledges it.
