Locke had a scar over one eye on Lost that remained for at least the first season of the show, though my fuzzy memory can't remember if it stayed after that. Spike on Buffy had his eyebrow scar. While it was James Marsters' natural scar, the makeup team deliberately enhanced it, which I sure they were regretting after they found out the character wasn't getting killed off.

As far as animated fandoms go, Hudson on Gargoyles had his eye scarred permanently, though being a gargoyle with magical healing abilities, that was almost an inversion. On Wolf's Rain, it was subtle, but by the end of the series, the main wolves were really starting to show some wear and tear in their human forms. Kiba and Tsume especially had some permanent marks by the end.
