Do we know for sure that she didn't? She's never there when Sayaka's there, but that doesn't mean she never shows up period (and considering what happens later she might have been purposefully trying to give Sayaka time to be with Kamijo). There's also a few references to her having a lot of extra curricular activities and studying because of her parents having really high expectations of her--they're in middle school, so she's probably trying to study for high school entrance exams among other things--so it's possible that she just couldn't fit the time to visit Kamijo very much in the hospital. Which sounds bad, yeah, but AFAIK Japanese culture takes that sort of thing waaaaay seriously, so she'd probably be seen as a bad daughter if she didn't go along with her parents' wishes or blew off her obligations to visit Kamijo.
Why did she spring it out of nowhere as soon as he got out?
Why would she spring it on Sayaka (and Kamijo) while Kamijo was in the hospital? He clearly had other things to be worried about while in the hospital so a love confession to him there would've probably been in poor taste.
Why didn't she try to discuss things with Sayaka at all or ask how she felt about the "I'll give you a deadline" method?
This one, I'll give you. I'm not sure either. The best explanation is that maybe she was nervous, or worried that if she wasn't blunt and quick about it that she'd start a fight with Sayaka. Still wasn't the best choice though.
Fans argue that she didn't owe Kamijou to Sayaka, which is true, because people aren't prizes... yet in the anime, it's Hitomi who treats it like a competition for a prize, and is putting on a show of niceness by giving Sayaka a head start.
I agree with this too, but I think it might be a cultural thing. I've seen that sort of attitude--the "first to confess wins" attitude--in quite a bit of shoujo. It shows up in the Tokimeki Memorial Girls' Side games, too, you can have friends that become rivals if you chase after the guy they like, and if you don't get him to like you first they'll often end up as a couple with the guy just because they made their intentions clearer.
