I think the point though is that the cat ears are totally arbitrary? Like why do they randomly have cat ears that disappear to begin with...?

The manga relies quite heavily on the concept of innocence. Ritsuka, despite everything that happens to him, clings desperately to his childish ideas, even when he dives into more mature and twisted concepts like revenge, abuse, and how to handle someone as messed up as Soubi is. The cat ears are just a physical manifestation to reinforce the idea.

Ritsuka's teacher still has her ears, and she still acts like a child. She's stubborn, easily hurt, and completely confused over the fact that she likes Soubi even though she's terrified of him because he once made her cry. She even became jealous just because one of Ritsuka's friends managed to have a friendly conversation with him when she herself can barely dial his number to call him without tearing up.

A big part of the ears is that many of the characters in the series suffered big changes when they lost them. They can never get the ears back, just like they can never get back to the way they were before losing them.

Oh, yeah, I got all that --

but the cat ears are still arbitrary. It could've been like, a mark on their forehead, or you know, something not... animal-like?
