Yeah. When watching the scene it basically struck me as an ugly cry face. Which isn't unusual. Many people have ugly cry faces. My face looks distorted and ugly when I'm crying. Not everyone's a photogenic cryer. And someone like Sherlock, who doesn't deal with his emotions...it seems plausible that when he becomes unhinged he loses control of his facial features etc.

Or alternately: He's putting on a show and doing these faces to sound convincing for John on the phone. The performance works either way for me. And it's sad either way IMO.
