The biology might be purely fictional, but some of the potential implications, such as the idea that people have/should have rigidly defined places in society based on their sexual role, or the idea that rape is a biological imperative and thus not the rapist's fault, ARE reflective of problematic real-world ideas. So there are some genuine reasons to find the idea problematic from a real-world standpoint.

(Note: I'm not saying all A/B/O fics do this, or even most, just that there are baseline aspects of the idea that potentially reflect problematic real-world philosophies)
