Had nosebleeds all the time when I was younger too - and I did actually choke a couple of times when the blood went down the wrong way. It was not a pleasant experience, I'll tell you that much. AFAIK, it's the risk of choking that's the main reason you shouldn't tilt your head back.

Got so used to it that when I went on an overseas school trip when I was twelve and wound up getting a nosebleed into my breakfast, I was totally zen about it while all my friends freaked out. XD
