Seriously, I'm writing first time sex (usually), starring people who are either promiscuous or are reasonably likely to get pregnant, and they're always smart people. Either I have to have a long disclaimer, some dialogue addressing safe sex and STI/sexual history, or there are condoms. Guess which one I usually fall back on.

I don't usually write or read porn, if you couldn't tell. My suspension of disbelief is not very good for this sort of thing.

(Also, do you even watch porn? Most of it involves condoms when there is a penis involved, unless specifically listed as bareback or whatever.)

And? Most porn that you watch involves real people who don't want to get diseases. Most porn that you read involves fictional people who won't get diseases unless the author decides they will.

And? Characters being fictional doesn't magically make disease not a concern. Unless we're writing The Sims fanfiction, the characters don't really look up at the sky, confirm they're not going to get a disease, and then go about their business.

In short, if you're writing such 1D characters that they are not even close to approaching real person problems, I don't want that it my porn either.
