...When did realistic, in-character sex between people who would generally take sexual precautions "ruin the fantasy of the story"? Why isn't my OTP allowed to use condoms, even if it would throw me out of the story if they didn't?

What about people who are careful to make sure that all the participants in their porn scene has the correct number of hands at all times, so they don't write half of their OTP getting it on with an octopus by accident? Does that also "ruin the fantasy of the story"?
