I would be thrown out of the action if an author went into a paragraph long diatribe about *why* the characters are using condoms and etc., but it never throws me out of a fic when characters use condoms. I don't find their use particularly sexy, but I find the consequences of not using them decidedly so. I can handwave some sci-fi and fantasy fandoms where the characters don't use condoms (magic contraceptives! STDs have been eradicated in the future! etc), but I find it a bit weird when characters in fandoms set in the late 20th century through the present don't use them.

Exactly. I don't think I've ever encountered a fic where the characters stop and give a lecture about why they are using a condom. The closest I can get is the occasional fic where one character insists on using one while the other doesn't care that much or where they put off certain acts until later for lack of a condom, but that's never a huge road block and sex still happens. No big deal.
