I don't see what's confusing about it. Yes, sometimes people try too hard to include references to safe sex, but unless going bareback is important to the scene, or more realistic for the characters/setting, I don't see how condom use destroys the fantasy.

And not everyone who writes sex is writing about their OTPs, or about characters who are in monogamous, committed relationships. If I'm reading a fic about a prostitute and a client, for example, the characters not using a condom might pull me out of the fantasy. Maintaining fantasy in porn isn't just about ignoring reality--it's about controlling details that could affect the reader's enjoyment. And if it's a scenario where there's a significant risk of pregnancy or STIs, then some people will notice the lack of a condom.
