While we rightly point out that him being forced to perform cunniligus was rape, I seriously doubt GRRM thought of it as rape. He thinks of rape as penetrative fucking, like most people.

I think it'd be naive to assume that's the only instance of sexual abuse Reek had to deal with while under Ramsay's "care", though. One can only imagine what went down when he got castrated.
