Which makes a degree of sense, because there are some Britishisms that would be unfamiliar to Americans and confusing your viewers is not really the best idea. Most of my familiarity with British media is Doctor Who and early Primeval, so things like Jack Harkness using UK terminology and pronunciation (the way he says estrogen/oestrogen in the Torchwood pilot is glaringly obvious to me and sounds "wrong") despite his American accent is explained by being from the 51st century. And being in Cardiff for a long fucking time, though that makes me wonder why he hasn't picked up a hint of a Cardiff accent.

I don't really know if American characters on more realistic British shows tend to use, say, mobile instead of cell phone, to use your example in reverse, but I'd expect them to. It can be vaguely annoying if you know they're wrong, but odds are most of the viewership isn't going to notice. However, in the case of Holmes it is IC, even if not as much to blend in as, he spends enough time explaining things, maybe he doesn't want to explain his terminology as well?
