My process is I think a bit different than most people's. My partner and I are both writers, so in order to sort out stories, we bounce ideas off of each other and straighten out plot points - what's the story about? Where's it going? What do we want to see happen? - and make sure that the internal canon makes sense. Then, for me, I pop my headphones in and just write. When I'm going back to a story I'm already working on, I make sure to read over what I've written to get myself back into the mood of the story. I make some changes as I go, but mostly my goal is to add to my word count.

Sometimes if I'm stuck, I just take a scene I want to see and write it as its own separate thing. It helps sort out what I want to see happen and what I'm going to need to do to get there.

But the biggest thing is: just write. It doesn't have to be amazing. The story I'm working on right now had a stupid introduction, but it gave me the framework I needed to get the story going. Eventually, the first paragraph got nixed because the rest stood on its own, and that's perfectly okay! Editing will happen later, so don't get so hung up on making everything excellent the first time through. Write now. Rework later.
