And how was it supposed to be taken in the era before electric ear thermometers? There were two choices, ass or mouth and you can't risk a mentally ill person biting down on a mercury thermometer so ass it has to be. Or you could just let a mentally person go without checking to see if they have a fever that is aggravating their condition. Mentally ill find it hard to articulate if they are physically ill, so the first clue is often an increase in agitated behavior. Best way to check for a fever is a temperature measurement, which brings us back to the old mercury stick.

Agitated behavior is still a red flag in the mentally ill for things like UTIs even today, just these days we have less invasive and safer ways of checking things like temperature.

It was extremely clear from the love of grabbing the lube and the sadistic pleasure of the orderlies as they pulled the person off to a private room, that the temperature was being taken was actually code for raping. The whole point of the novel was to show the hospital as being an abusive environment, where the staff didn't care about the wellbeing of the patients.
