The only people going outside like that here tend to be demented old folks who are lost, so that would be the default reaction.

Also northern Europe, and now that you mention it ...

I suppose when I read "pajamas", I assumed that meant pajama pants (and long-legged ones!) with an ordinary t-shirt, or at least with a jacket over the pajama top. That might make people do a (mostly discreet) double-take here in Sweden, at least if the pants had very noticeable print, but security wouldn't even blink.

Anyone walking into a grocery store (or further from the house than the mailbox, really) in full pajamas and nothing else (nothing sensible, that is) would get a lot of stares.

Northern Europe again, and I've done the one you mention: pajama pants (tartan, I might add), t-shirt, my heavy coat and my boots. It was ten at night and we'd run out of milk, the shop was ten minutes away and no-one in my family can function without tea in the morning, which requires milk. Rather than have to get up earlier in the morning and run to the shops and back in the craziness of the school-and-work run, I just decided screw it, boots and coat over my PJs, and walked the damn ten minutes to the shop. I got a couple of looks, mostly because of the tartan, I think, and otherwise was perfectly fine.

So long as everything's covered, you're not freezing yourself to death and you're not in your underwear, I don't see the problem.
