There are episodes of all 3 series post-holodeck-invention that involve it without anything going wrong.

They either use the holodeck as a side-detail, or use them as an excuse to play with another genre, rather than kicking off a standard-Trek plot. And even some of the ones where something does go wrong, it's not the holodeck malfunctioning - like the DS9 episode where they had to play out a heist movie because the holosuite was used to store the cast's patterns after a transporter accident, or the Voyager episode where the alien takes the holonovel that Paris was running to be reality and they have to keep it up to deal with him.

It's a Bond movie, not a heist novel. And I counted it as a "holodeck malfunctions" episode - I would count any episode where the safety protocols are disengaged, or where the characters are unable to leave the holodeck, as a holodeck malfunction episode.

But yes, it's true, they do use them for color sometimes.

You're conflating two episodes- "Our Man Bashir" is the Bond-style adventure where they have to restore the senior staff from a transporter accident. "Badda-Bing, Badda-Bang" is the heist story, and it's a pretty dumb episode. I guess it does count as a malfunction episode because the computer can't remove the gangster characters and it seems like they could actually "kill" Vic.
