According to TV Tropes, "There is a genre of fanfiction called "whump", which almost exclusively involves having the bad guys kidnapping one of the guys and doing painful things to them, often leaving them emotionally scarred, all so that they will be saved in the end."

So I guess... that as a fetish...?

I think the thing that they get off to is the savior element of it all. Like, "Look at me, I'm so amazing, I can save a completely helpless person from a completely ruthless enemy, and then that person needs comfort, so I can do that, too." There's usually no small amount of healing cock/pussy involved as well. So, the reader can get off on the character getting hurt and can still feel proud because the character is in a good position at the end despite all the crap that happened at the beginning.

Well, it is hurt/comfort, but more extreme and almost entirely physical. (For me angst would be emotional hurt without much resolution, hurt/comfort would be physical and/or emotional hurt with resolution, and whump is a subcategory of hurt/comfort with extreme physical and possibly emotional hurt with a resolution.)
