Not nearly as serious, but this reminds me of messing around with an air rifle when I was eleven. Really powerful pistol that my friends dad had. Well we're screwing around with it and the pellet in the chamber drops out.

I look down the barrel to confirm this is the case, finger on the trigger the whole time, and then working under the assumption the chambers empty think 'heh I'll give my little bro a proper scare' and point it at his eye from a foot away. For god knows what reason at the very last second I centre it on his forehead instead and fire.

He's crying and bleeding everywhere and I learnt a lifelong lesson in gun safety having been a split second away from half blinding my brother or myself.

My uncle is actually blind in one eye from a childhood incident involving a air rifle (or possibly a bb gun, not sure). He and his brothers were fooling around thinking it was "just" an air rifle and wouldn't really hurt anyone.
