I think depends on whether you're using the Greasemonkey version or the browser extension version. The browser extensions will block a post if a blacklisted thing comes up anywhere in the post, but the Greasemonkey version will only block it if it's in the tags.
I've only used the Firefox add-on version, so I can't confirm this, but it's what I've heard from a friend who's used both.

I use the greasemonkey version for firefox, and it catches words in posts (or it used to... it's been spotty lately, but I need to update). I actually had to whitelist a few usernames because they had words in them that I'd blacklisted and it was blocking all of their posts.

I'm actually glad it blocks usernames. There is somebody I don't want to unfollow because they'll know it was me and they are the kind of person to get really upset, but at the same time I don't want to see their posts. So I just block their username and all their posts disappear. Win win.
