Who knows? The Doctor might do that as a means of self-abuse. He's been through a lot. It's easy to forget that when he's being silly and saying funny stuff.

Now I want to see fic where Donna catches him purging and just holds him while he cries and deals with his self loathing, and she's not judging or anything.

Seriously, this. I went to a family wedding, and I was 120lbs(I'm 5'4" so that is completely healthy) and the entire night every single person kept telling me I was disturbingly skinny, I need to eat, blah blah. It's fucking angering.

My husband is about as thin and I promise he's not anorexic. Him and my son have this amazing metabolism. Neither has to even think about food because it just magically disappears when they eat it. When my son goes through a growth spurt he eats and eats and eats and I think WHERE IS THE FOOD GOING?! Even as a baby he didn't have much baby fat and it wasn't for lack of food. :P

I assume you mean the character, not the actor, right? Because he's really not THAT skinny, but given the character's self-destructive tendencies...well, it's not unbelievable, although I don't recall any actual evidence or counter-evidence.

Although being obsessive about being in control of his body is pretty in-character -- he does it already in a far milder context, with his clothing. I don't think he ever unbuttoned his shirt or undid his tie once if he was wearing one, and the only time, as far as I remember, that he ever took his suit jacket off in front of another person was the rooftop scene with Donna in The Runaway Bride.

Does...does the Doctor actually need to eat at all? I'm under the impression his humanoid bodies are pretty static until he gets hurt bad enough to regenerate (with reasonable exceptions, like that time the Master turned him super old or whatev), am I getting this right? I only watched like a season and a half sporadically

Well, he gets hungry. And he does age - slowly. And he's been shown to get sick. Whether he'd starve as fast as a human is up to speculation, but it's a fair bet that his body is not totally static, just...resilient and able to widthstand a lot.
