It's fairly straightforward to be honest. Use an art program -- paint will suffice, but you're better off torrenting an old version of Photoshop (I recommend CS2) if you can find one or GIMP which is free but requires that you hate yourself. There's plenty of tutorials on how to use all three, but you'll never really learn if you don't fuck around a bit.

Find a picture that may or may not relate to the subject of your secret. A couple years ago someone posted a secret about murdering someone who didn't like Twilight, and included a picture of a shovel. (pretty sure that's the only one that ever got the "take it to the cops" listing in the failbar)

Slap your text on. Make sure it's readable (Photoshop has a "blending" option that allows you to put borders around your text, I find this helps when you have text going over multiple contrasts.) Make sure the pic is in the appropriate size (below 600x600 pixels) and save it as a jpeg or png. Toss that shit up on imgur and submit!

Alternately you can just upload the picture you want unedited onto imgur, select the "edit" option before you upload, and use the advanced tools the edit page gives you.

Paint.NET is a pretty good one, and it's free. That's what I use.
