Digital - Paint Tool SAI

Paper, pen and pencil.

Although sometimes I use Photoshop to do the trick.

Mechanical pencil with 2B or lighter lead. Micron pens, usually Graphic 1. Erasers. Erasers. Erasers. And Photoshop for clean up.

I'm not sure what you mean by outlining..
I always sketch by hand with pencils, then to "Ink" the sketch I either use
Calligraphy pen, and bottled ink if I'm doing it by hand, or I scan it and use the GIPM paths tool, and ink tool. I tend to use GIMP more these days, I get a much better result.

Pencil, then micron pens or dip pens (mainly a g-pen) for inking. I also use brush pens now and then.

I usually do the initial linework with a .5mm mechanical pencil, then scan and ink and colour in Photoshop.

Pencil for sketching, then I usually scan it into the computer and use SAI. But I use black ballpoint pens for traditional stuff. I like these a lot.

Depends on what effect I want for traditional pen paper stuff, so it varies between a brush pen for thick lines, a thin brush with acrylic or India ink for a slightly different look in terms of varied line weight, and .2 ish microns if I'm feeling for plain lines.

With digital, paint tool sai is pretty much perfect, but I avoid the vector line tool unless I need it for something more static and lifeless like... poles. Also, I'm digging the ink tools in MyPaint; if it let me save into something like gimp,I would probably use it more often for inking.
