One common addendum to that headcanon is that he doesn't actually know it's happening (and may not even be aware of the third eye's existence). This is not true in my brain, but I sort of love the idea.

That's a cute idea. I think you could even connect the third eye to the sentient tattoos thing: If Cecil got tattooed in Nightvale, it wouldn't be any supernatural talent he'd been born with but the probability that a tattoo in Nightvale might get a little funky is quite high.
That's why I like the eye as a tattoo at the back of his neck.
