Dean was basically an atheist at the time. They didn't know that demons were tortured human souls either. IMO, if he thought about what happened when people died, it was that they either became tortured spirits lingering on earth or they just dissolved into nothingness.

Either way, the point wasn't that Sam was suffering in Hell. It was that Sam was dead. Sam was dead when he had had it drummed into his head since age 4 that he had one job and that job was keeping Sam safe. Dean already felt like he'd let John down - he couldn't stand failing Sam, too. That alone was more important than where Sam was. I bet Dean would have made the same decision even if he'd been 100% confident Sam was in heaven.
